What is the role

Helping people to remain in their own homes and get the support they need and coordinating a person’s care across all areas.

What does it involve

  • Working with people with a range of needs
  • Dealing with issues such as social isolation and preventing necessary admission to hospital or a care home
  • Working with external stakeholders to create a personalised care plan for the individual
  • Organising and managing support/care staff
  • Monitoring service users progress

Salary range

from £24,500 to £29,000

Entry qualifications required

Qualification requirements for these roles vary with some expecting professional qualifications, degrees or a minimum of a level 4 qualification. You’ll need to have strong English and number skills.
