Support people to live independently, often following an illness or accident and helping them access support. This can be for a set period of time or until the goals have been met.
What does it involve
Carrying out assessments within the community to identify what care and support people need
Working with other professionals such as social workers and occupational therapists to make sure people get the right help
Providing advice about how to use specialist equipment
Salary range
from £25,000 to £35,000
Entry qualifications required
No formal entry level qualifications, but need strong english and maths such GCSE grades A–C in Maths and English.
Holding or working towards a Level 3 Diploma or Apprenticeship in Health and Social Care is desirable.
